1. Proof of Insurance/Certificate Holder: When the location requests proof of insurance -Request a liability certificate naming the location as Certificate Holder.
   
   
2. Additional Insured: When the location expressly requests to be named as an additional insured on the NFHS’s liability insurance policy. Request a liability certificate naming the location as the certificate holder and clearly indicate that the location must be named as an additional insured on the NFHS’s insurance policy.
